  • The Mitsubishi Dealership Employee Identification Number (MDEID) is a unique identifier generated by MMNA to replace use of Social Security Numbers in order to reduce the risk of identity theft.

  • Your MDEID is required to enroll in Diamond Rewards.
  • Contact your dealership's MDL Administrator if you have not been assigned a MDEID or have forgotten your MDEID.

Cardholder Agreement

The recipient who submits this form is receiving a Mitsubishi Diamond Rewards Visa Reward Card and agrees to be bound by the Disclosure and Master Agreement delivered with his or her Mitsubishi Diamond Rewards Visa Reward Card, and that Mitsubishi or JNR Incorporated may change or amend the terms of the Disclosure and Master Agreement or discontinue this service at any time.

  2. The Legal First and Last Name provided is my correct full legal name and
  3.
